Educating Patients on Medication to Reduce Hospital Readmissions

There are times when patients do not complete their dosage and undergo re-hospitalization for treatment is they miss their medication. Lack of awareness or the knowledge about the medication is the key factor behind re-hospitalization. The ACC Quality Summit in New Orleans, shows that if patients understand their medications and take them as instructed, a high chance for reducing avoidable readmissions exists. After implementing this program, patient readmission reduced from 8.2% to 3.4% in the second quarter in 2018.

Medication Information to Change Patients’ Perspective

Heart diseases can be overwhelming for patients, as they have to deal with new medications and restrictions after leaving the hospitals. Although doctors inform patients thoroughly about their medications, they might have difficulty understanding and following these instructions.

Due to these conditions, Olathe Medical Center in Olathe, Kansas came with a new plan to mitigate this issue. The American College of Cardiology’s Patient Navigator Program equally participated in this study. In this study, the researchers developed solutions taking into account patients’ perspective on hospital discharge and follow-up experience.

Study finds that patients fell they have left the hospital with little preparation for the discharge plan of care. Another thing is that hospitals lack follow up care at the time of discharge. Limited communication by the hospital authorities led in rise of these conditions. Additionally, expectations are unclear regarding their recovery and needs. Clinicians give patients better medication reconciliation and instructions to help them understand their medication regimes.

To address these situations, the Olathe Medical Center came up with a medication education communication process. In this system, bedside nurse will review any new medications included by any family member or caregiver. Before leaving, patients will write out medication information on a visual aid form called a medication log with all medication details. The floor pharmacist educates patients about the importance of medication, side effects, and other dos and don’ts. Cardiology nurse navigator, outpatient cardiac rehab staff, and floor pharmacists each will perform one task.
